Established in 2021, WeCare Wellness is an Australian owned, private company that provides Aged Care, NDIS Care and Nursing Agency services throughout Sydney to a diverse range of clients. We’re an Approved Aged Care Provider committed to deliver unparalleled quality and flexible services to our Home Care Package (HCP) and private fee for services clients.

WeCare Wellness Aged Care services help with a range of personal, clinical and lifestyle needs while providing welcome companionship for ageing adults. Services include basic DA, PC and SS service to advance community Nursing services, as well as PT and OT service etc.. 

We pride ourselves on delivering tailored care to enhance the quality of life for our clients and their families. Our ultimate goal is to support clients to remain at home and engaged in their local community. ABOUT THE ROLE

Reporting directly to the Operations Manager, you will provide effective and high-quality care coordination and management services to support individuals living in their homes. Specifically, you will be responsible for facilitating care solutions for individuals and their families to ensure consistently high-quality care services to our Home Care Clients. You will be supported by our local nursing team and health professionals.

You will be responsible for:

    • Provide in-home assessments, referrals, care plan reviews & goal planning for consumers
    • Client budget management & reporting
    • Excellent time management with client caseload
    • Respond & communicate effectively with clients re: changing care needs
    • Relationship management with internal & external stakeholders
    • Possess empathy & understanding towards the needs of older people

To be successful in this role you will have the following skills and qualifications:

SKILLS AND EXPERIENCES

    • Bilingual and able to speak English and Mandarin is essential
    • At least 2 year’s experience in aged care and Case Management
    • Demonstrated understanding of relevant Commonwealth legislation as it applies to Home Care Packages and the Aged Care Quality Standards.
    • Ability to develop and maintain excellent working relationships and demonstrated customer service skills.
    • Knowledge of budget management and fees and costs associated with Home care packages
    • Understanding of the interface between CHSP and HCP  
    • Ability to work independently and be a resilient problem solver
    • Demonstrated strong awareness of risk and solid report writing and administrative skills.
    • Proficient with client management systems, Microsoft office and government reporting funding portals
    • First Aid Certificate
    • Hold a current valid driver’s license.
    • A clear national police check upon role offer
